If you require any vaccinations relating to foreign travel you will need to complete a Travel Health Questionnaire.

It is important to complete the Travel Health Questionnaire early as possible – at least 8 weeks before you travel. You can either collect a travel questionnaire from our reception or the link below, once the form has been completed and verified by our Practice Nurse we will advise you on the vaccinations required. These vaccines have to be ordered as they are not a stock vaccine. You will then be invited in for an appointment with the Practice Nurse at least 4 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
