Coronavirus Help & Forms

This page is dedicated to providing help and advice to all our patients during the Coronavirus outbreak. On this page you will find links to forms for your employers and advice on how to get help within the community if you are self isolating.

Advice on Wearing Facemasks

Facemask Exemption Letter – Whilst we appreciate that these are challenging times for our patients, it is not the responsibility of a GP to issue an exemption letter to an individual from not wearing a face mask within various public settings.

Government guidance regarding exemptions suggests there is no requirement for an individual to produce evidence of exemption, such as a letter from a GP and therefore should be sufficient for an individual to self-declare this. In these circumstances, it is not a medical reason that requires a written letter from the GP for those who do or do not, fall under the list of exemptions.

Coronavirus MED3 Forms

In most cases, patients do not need a MED3 form from their GPs, they can self-certify for seven days and only if ill should they require any form of certification.

Patients can use the following form to give to their employers should they need to self-isolate due to Coronavirus Self Isolation for Covid-19

The practice will not provide a medical certificate for patients requiring self isolation for coronavirus.
