Sherrington Park Medical Practice already operates a personal list and patients are familiar with having a personal GP. The named GP has overall responsibility for the care and support that the surgery provides for them. The named GP will also work with other health and care professionals to ensure that those patients over 75 who are vulnerable and at risk of admission to hospital has a care plan which meets their individual needs.
