New Patients

New Patient Registration 

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

Unfortunately, we are unable to accept registrations from anyone who lives outside of our catchment area so please contact us to check before trying to register or click here to use the practice.

Catchment Area

You will need to complete a registration form (GMS1) AND a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients on regular repeat medication will be required to have an appointment with one of our GPs to review this medication and if appropriate a repeat template will be created.  To avoid running out of medication, it is advisable for new patients to obtain at least 1 months’ supply prior to registering with us, should there be any delay in obtaining an appointment with a GP for review.

Medical treatment is available from the date of registration. Please contact reception for further information.

Patients with Particular Needs

Our surgery is accessible to patients using a wheelchair. We also have a parking space reserved for patients displaying a disabled sticker. For patients with hearing difficulties there is a hearing loop available.

We can arrange interpretation and translation services in person or by phone for patients who do not speak English. Please let us know if you need this service when booking an appointment.
